Purchase Order and Shipping Instructions

Completion of the ISF Submission Form is MANDATORY for ALL ocean shipments. This worksheet MUST be accurately completed and sent to both the Expeditors and Goodyear ISF mailboxes no later than 96 hours prior to vessel cut off. Your failure to provide on time/accurate ISF or respond to our inquiry for more information which produces demurrage/detention charges and/or penalties by U.S. Customs & Border Protection will be debited to your account.

Goodyear is a Tier III C-TPAT member and as a result, C-TAPT membership and/or compliance of our suppliers is critical to our operation. All non-C-TPAT members are subject to as risk assessment process. For more information on C-TPAT, click here.

This document is to be completed when a supplier is shipping fabric into the United States. The questions asked will allow the broker to correctly classify the fabric, which is critical.

This form is provided in the event the product or products being shipped into the United States by the supplier were produced in multiple countries.

Single/Multiple Country Declaration

This form is required if the product is subject to controls under the Toxic Substances Control Act.
